JR High Vaccine Clinic at IRMC

Dear Families,


Indiana Regional Medical Center has reached out to the District to make the COVID-19 vaccine (Pfizer-BioNTech) available to any interested student ages 12 to 15 years or older and anyone else who might be interested.  The attached paperwork will need to be completed and submitted at the time of the visit for the first dosage. The vaccine would be administered at the Kovalchick Center on Tuesday, June 1st (1st dose), June 26th (2nd dose). You will need to provide your child transportation for their 2nd dose of the vaccine due to school being out of session for summer break after June 4th.  IRMC will be administering the Pfizer vaccine for those interested individuals.  To assist IRMC in planning, we are asking all interested families to complete a brief survey by the close of business on Monday, May 24, 2021Click here to complete the interest survey.  


To help assist interested students in receiving the vaccine, IASD will bus students to and from the Kovalchick Center. We are estimating that the total time out of school (busing and receiving the shot) will take about an hour from start to finish.  A health professional from IASD will accompany students to and from KCAC.  The Jr. High school administration team will handle coordinating these efforts.  Unfortunately, we can only bus students at the current time, not family members.  The consent form needs to be signed by the parent/guardian.

 

The students will need to bring the following documentation:

  • Photo ID
  • Complete consent form signed by the parent/guardian
  • Insurance card

I apologize for the short notice. Please understand that this is your choice as a family to decide whether or not you want to receive the vaccine.  We are merely assisting those who are interested in receiving the vaccine.  As always, we appreciate your support, grace, and compassion now and throughout the year.  Please call the Junior High School at 724.463.8568 should you have any questions.
